Giving you one to one support to make realistic, positive lifestyle changes.
You will have a 1 hour health check, which will include a discussion of your eating patterns, exercise levels, stress levels & alcohol intake. Also checking your blood pressure, weight, height, body mass index (BMI) and cholesterol measured to give you a starting point. We will then work with you to produce a personalised support plan just for you.
Your Health Trainer will keep in touch to offer support and arrange follow-up appointments to see how you are getting on and offer you practical support for up to 12 months on the following:
- Healthier eating
- Increasing your activity levels
- Managing your weight
- Improving your health and wellbeing
- Reducing stress
- Supporting with alcohol/smoking cessation
- Signposting to other support services/community groups.
How do I access the service?
Anyone aged 16 and over can access the service either by self referral, or by being referred by a health professional. You must either live in St Helens, be registered with a St Helens GP or work in St Helens.
