Information for Professionals

Our integrated wellbeing service is commissioned through St Helens Council Public Health department and delivered by City Health Care Partnership CIC (CHCP).

We support and empower local people, children and families to have a healthier lifestyle and improve their health and wellbeing. With services and support ranging from pre-conception, antenatal, postnatal, early years, children and young people (0-19) and adults through to older people, our goal is to help people make long-term behaviour changes to improve their health and wellbeing. We use a tiered approach from light touch information and engagement through social media or at local events, through medium touch planning, support and intervention, to more intense support for hard-to-reach communities or people with additional needs.

The evidence shows that early interventions can support healthier life choices and outcomes and we offer a choice of methods including telephone, face to face, virtual, group sessions and family activities. We pride ourselves on a non-clinical, down to earth, peer to peer approach and work to identify different communication needs and learning styles to support a more sustainable behaviour change outcome.

Small Grants

Through our charity, City Health Care Partnership Foundation, we give extra support to the communities we work in by offering small grants to voluntary and community groups who are helping to improve local health and wellbeing.
